ASSESSMENTS AFTER ENROLLMENT
PATHS- PERSONAL ASSESSMENT TOOL FOR HEALTH AND SERVICES
PATHS is a Person-Centered Planning assessment tool completed with you and for you used to ensure that the Life Plan reflects and supports your vision of a happy and meaningful life. Also, it helps the Care Manager/Care Coordinator uncover unmet needs, so they are able to offer supports and services that would help meet that need.
The PATHS
- Assesses all areas of a person’s life, including communication, providers/medical needs, mental/behavioral health, future planning, independent living skills, medications, education, and employment.
- Help to define abilities, needs, and your personal goals and safeguards.
- Helps you and your Care Manager/Care Coordinator develop your plan of supports and services that will build the skills needed to reach your goals.
- Helps you and your Care Manager/Care Coordinator understand the needs and health of individuals on a larger scale, so the CCO can advocate for services and supports that are lacking, such as dental care.
- Is viewed with you and your team at a minimum annually, and more frequently if desired, or as needs and wishes change.
