CARE MANAGEMENT

EMPLOYMENT

Integrated employment in the community is a wonderful way to stay active, participate in community activities, develop your social and adaptive skills and explore opportunities that might not have otherwise been considered. Your Care Manager/Care Coordinator can explore a variety of options with you, if you are interested in pursuing employment.

 

Adult Career and Continuing Education Services- Vocational Rehabilitation  (ACCES-VR)

  • ACCES-VR is not an HCBS Waiver service. It is offered through the NY State Education Department.
  • Vocational rehabilitation counselors assist in developing a plan and reaching employment, educational or vocational goals.
  • Training, skill building, education, rehabilitation and career development are provided to help you attain and maintain employment.

 

Supported Employment (SEMP)

  • Job skills training and coaching offered through the OPWDD Waiver, to assist in finding and maintaining competitive employment in the community.
  • On-the-job support is also available, once employed.
  • This support usually follows ACCES-VR and requires limited job coaching to maintain employment. • Programs may include an internship or job shadowing, which may lead to permanent employment.

 

Pre-Vocational Services

  • The majority of these services are provided in community-based job sites. However, some agencies still operate site-based prevocational centers as well.
  • Workplace environment social and communication skill building.
  • Does not include on the job training tasks.
  • May not provide paid employment during training.

 

Pathways to Employment

  • One-year program to develop a plan for achieving competitive, integrated employment at or above the New York State minimum wage or explore unpaid or volunteer opportunities.
  • You will develop a realistic career path that interests you, and develop a plan to get there
  • Staff will work with you on your plan and assist with skill building to help you reach your goals.
  • Some potential paths may include job shadowing, internships, and volunteer work
  • Programs include learning how to build a resume, apply for job and volunteer positions, and prepare for interviews.
