SELF-DIRECTION

Self-Direction provides increased flexibility in choosing the supports and services that you desire, the staff that you choose to work with, and the schedule that best suits your needs, enabling you to live life the way you want, with more control and independence.

 

Self-Direction Service Options

 

Community Habilitation: One-to-one training to help build the skills needed to live independently at home or in the community.

 

Group Day Habilitation (Direct Provider Purchased): Enhance skills to live independently, participate in the community, develop interests and build friendships.

 

Prevocational Services (Direct Provider Purchased): Help building the skills and strengths to overcome obstacles to obtaining and maintaining employment.

 

Pathway to Employment (Direct Provider Purchased): Help to identify an attainable career path and plan. You can job shadow, intern, volunteer, learn how to apply for positions, prepare a resume and interview.

 

Supported Employment (SEMP): Supports to help you maintain a paid competitive job in the community.

 

Respite: Help available to you should you have a caregiver that is unable to assist for a specified time.

 

Housing Subsidy/Individual Supports and Services (ISS): Funding to help with rent and utilities in the community.

 

 

Assistive Supports: Adaptive devices, aids, appliances, etc. that enable you to increase or maintain independence at home.

 

Live-In Caregiver: A caregiver, unrelated to you, that resides in your home to support your ability to stay safely in your home.

 

Paid Neighbor: A neighbor of your choosing who is available to you to provide help and assistance should you need it.

 

Support Brokerage: Assists you in your plan of services, budget for these, recruitment of staff and your staff action plan.

 

Fiscal Intermediary: Assists you in maintaining your Self-Direction Budget, hiring, maintaining and paying staff as appropriate.

 

Individual Directed Goods and Services (IDGS): Equipment or supplies identified as a need to help you achieve your goals, but is not covered by traditional state funded services, such as classes, trainings, health clubs, and transportation to name a few.

 

Other than Personal Services (OTPS): If Self-Directing with Budget Authority, you can choose up to $3,000 from your budget to cover additional goods or services not covered by Medicaid, if they relate to your identified goals. 10 Steps to Self-Direction

  1. Let your Care Manager/Care Coordinator know you are interested in Self-Direction
  2. Care Manager/Care Coordinator submits Service Amendment Request Tool (SART)
  3. Service Amendment Request Tool (SART) approved by OPWDD
  4. You and your family attend an OPWDD Self-Direction Orientation
  5. You select a Self-Direction Broker and Fiscal Intermediary
  6. You complete a Broker Agreement and a Start-up Budget Template
  7. You, your family and your broker map out a budget
  8. Broker sends budget to OPWDD for approval
  9. OPWDD approves the Self-Directed budget
  10. Circle of Support meets to launch budget, services begin
