PERSON-CENTERED PLANNING

What is Person-Centered Planning?

The Person-Centered approach is the core of a number of assessments used to gather information about you. Results help identify the most useful and appropriate services that will build on the specific abilities and skills to help you reach your goals.

 

Person-Centered Planning

  • Seeks to understand you, as an individual.
  • Is directed by you, based on your wishes and goals.
  • Helps others learn how you want to live.
  • Helps identify supports to help you move toward a life that you find meaningful and productive.
  • Empowers you by building on your abilities and skills that will help you lead your best life.
  • Enables you to construct and articulate a vision for your future, consider various paths and opportunities you can pursue to accomplish this vision, engage in decision making and problem solving, monitor progress, and make adjustments as needed in a timely manner.

 

Some questions your Care Manager/Care Coordinator may ask:

  • What are some goals you have for yourself?
  • What are your likes and dislikes?
  • What challenges impact you most?
  • What do you like about yourself?
  • How do you communicate?
  • What is a priority you have right now?

 

How is this information used?

The information gathered is reviewed by you, your Care Manager/Care Coordinator and your circle of support. This becomes the foundation of the person-centered care plan, called the Life Plan. Appropriate supports and services are identified, based on the information. The goal, always, is to seek services that empower you by developing or enhancing the skills needed to achieve your desired relationships, community participation and ideals with dignity and respect.
