EQUITABLE HEALTHCARE

Health equity means that you have access to healthcare no matter where you live or what resources you have (e.g., access to transportation, access to the internet, and required technology for care provided through telehealth services). In addition, you can access resources in your community that support you with social determinants of health, such as food, safe recreational spaces, and housing.

One of the roles of the Care Manager/Care Coordinator is to support you with navigating health and behavioral health systems by:

  • Assisting with doctors’ visits, emergency room visits, hospital discharges
  • Checking in with you to find out the outcome of a medical appointment and ensure that you have a good understanding of what happened during the visit.
  • Supporting you with making appointments, or any other follow up needed, and with identifying someone in your natural support circle who can help you with any areas of healthcare that are difficult for you.
  • Educate you about resources in your community.

 

What happens if you become seriously ill and are unable to make decisions about your medical care? Who would you choose to make such decisions on  your behalf?
