Self-Direction provides increased flexibility in choosing the supports and services that you desire, the staff that you choose to work with, and the schedule that best suits your needs, enabling you to live life the way you want, with more control and independence.

Steps to Self-Direction

01

Tell your Care Manager (CM) you’re interested in Self-Direction.

Join the Self-Direction (SD) webinar. Your CM can share the webinar schedule.

Wait for OPWDD’s approval to move to the next step.

Choose your Support Broker (SB) first and then choose your Fiscal Intermediary (FI).

Meet with your chosen FI for an intake/enrollment meeting.

Once you and the FI agree to work together, update your Life Plan to include your Broker and FI in Sections IV & V.

Submit your Startup Budget to OPWDD.

Once your Startup Budget is approved, complete and sign a Broker Agreement and start planning your full budget.

Update your Life Plan with a Life Plan Addendum to match your Startup Budget.

Submit all required documents to OPWDD.

Once your budget is approved, attend a Launch Meeting.
