What to Expect from your Care Manager/Care Coordinator
Based on your needs, OPWDD sets minimum requirements for contact with your Care Manager/Care Coordinator, including in-person visits and Life Plan meetings. Your Care Manager/Care Coordinator is available to meet more often, if you choose.
Life Plan meetings are required to be held every six months. The annual Life Plan meeting must be held in person, and the circle of support must participate in the meeting.
