What to Expect During Enrollment
1. SERVICE REQUIREMENTS
Must meet qualifying diagnoses, and
- Disability occurred before age 22
- Disability can be expected to be indefinite/permanent
- Disability causes a substantial handicap to adaptive abilities
2. CONTACT AN ENROLLMENT PROFESSIONAL
Contact an Enrollment Professional at your CCO of choice to guide you through the process.

3. VIEW FRONT DOOR VIDEO MODULES
This training is a required step in the process. The videos provide introductory information about the services provided by the OPWDD system.
4. DETERMINE ELIGIBILITY
OPWDD will need to review documentation to determine your eligibility for services. Click here for a list of documents that will be needed.
5. APPLY FOR MEDICAID
Work with the CCO and your Enrollment Professional to apply for Medicaid. Medicaid coverage is required to pay for Care Coordination and the majority of OPWDD services.
6. ENROLL IN A CCO (CARE COORDINATION ORGANIZATION)
Enroll in your CCO of choice to be assigned a Care Manager/Care Coordinator who will work with you to connect your needs and wishes to appropriate supports.
7. APPLY FOR OPWDD WAIVER SERVICES
Work with your Care Manager/Care Coordinator to develop a plan to meet your needs. This may include applying to enroll in the Home and Community Based Waiver, or finding other services in the community that will help you live your best life.
